Nursing Home Diversion Provides a comprehensive assessment to applicants seeking admission to nursing homes, Dom care or personal care homes and assigns a decision recommending the most appropriate and least restrictive level of care for the individual. When appropriate, some persons are maintained in the community and provided care management and/or other intensive in-home services as needed.
Personal Care Provides hands-on care to functionally dependant older persons who need assistance with bathing, dressing, taking medications, etc.
Attendant Care/Personal Assistance This program provides extensive long term care help for physically disabled adults who need hands-on personal care and help with activities of daily living at home.
Care Management Provides an assessment to arrange for various needed services for older adults. It also serves as the method of access to the long term care system for both the older person and family.
Adult Day Care Provided in a licensed facility this service makes available therapy, meals, recreation and support to families who are responsible for the care of a dependant older person.
Domiciliary Care Provides placement in a home like atmosphere for adults who need help with some aspects of the activities of daily living. Dom care is available for individuals 18 years of age and over with disabilities.
Home Health Provides physician-ordered nursing, care, home health aide or various therapies in the home.
Respite Care The agency has agreements with local hospitals and a personal care home to provide respite for older persons whose families are the primary caregivers. Service is also available to provide overnight supervision for an older person who would be considered at-risk if left unsupervised.
